Damien Center empowers communities and persons affected by HIV/AIDS in the state of Indiana by being a leading resource, provider, and advocate for comprehensive HIV/AIDS care, prevention, education, and related services. Damien Center, Damien Cares and Damien Pharmacy provide client-centered social services focused on care coordination, clinical and pharmacy services, mental health, housing, and nutrition to the Indianapolis community.

Position Summary:

The Primary Care Physician (PCP) works within the Damien Cares Clinic assessing and treating common infections and diseases for adults and youth in an outpatient setting. The PCP treats patients living with HIV but does not offer infectious disease management or ongoing HIV treatment planning. The provider will consult with collaborative infectious disease medical providers to assure quality care is provided, and when appropriate, refer to external specialty care or other needed medical care. The PCP will work with LGBTQ+ populations, individuals living with HIV, and individuals who may be facing addictions, mental health concerns, and unstable living conditions. The PCP works as a member of a multidisciplinary team, including psychiatric, mental health, case management, and other supportive services. Must be a strong advocate and supporter of Advanced Practice Providers.

Duties and Responsibilities:

• Independently provide care for patients with acute and chronic illnesses encountered among low-income patients, many of whom are living with HIV or identify as LGBTQ+.
• Take full accountability for patient care and outcomes and appropriately seek consultation regarding HIV and infectious disease care from specialists when needed, while remaining involved and responsible for the detailed care of the patient.
• Responsible for assessment, diagnosis, treatment, management, health education, care coordination and documentation for patients with acute and complex chronic health needs.
• Consult and collaborate with infectious disease providers to ensure effective HIV care and treatment as well as collaborative care and established boundaries.
• Provide PrEP, nPEP, and primary care to patients enrolled in the PrEP program.
• Serve as collaborating physician with Advanced Practice Providers and review notes as assigned.
• Provide culturally sensitive care to transgender and non-binary patients, offer hormone treatments to patients as appropriately assessed, and coordinate care with specialty providers serving transgender and non-binary patients for services outside of professional scope.
• Obtain and accurately document any relevant health history.
• Perform comprehensive histories and physical exams, identify normal and abnormal findings, including physical, cognitive, and psychosocial status.
• Order and interpret laboratory and diagnostic tests.
• Develop pharmaceutical and non-pharmaceutical treatment plans.
• Educate patients on disease and treatment plans.
• Monitor effectiveness of treatment plans.
• Document all interactions within the Electronic Medical Record (EMR) within 48 hours of interactions and in the format as established.
• Coordinate with the billing team to maximize billing, accurate notes, and adjust if needed.
• Collaborate and/or consult with other health care providers as appropriate.
• Educate, counsel, and assist patients and their families regarding current health problems, health maintenance and preventative behaviors.
• Acquire information on the most current developments and trends in the HIV/AIDS field through educational programs, in-services, professional associations, research and literature.
• Participate in quality assurance and compliance activities.
• Develop and implement best practices and standard of care for patients requiring infectious disease consultation.
• Performs special assignments and other work, on an as-needed basis.

Education and/or Experience:

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
Current, active MD or DO licensure in State of Indiana required. A minimum of 3-year clinical experience in geriatric, adult or family practice setting preferred. Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty is preferred, Board Eligibility is required. Once Board certified, PCP will maintain board certification in their terminal specialty by doing necessary MOC, CME and/or retaking board exams as required. Basic Life Support (BLS) certification required. Must have a current DEA number for schedule II-V controlled substances.

Knowledge, Skills, and Abilities:

Possess superior organizational and administrative skills. Excellent interpersonal and communication abilities. Excellent computer skills in a Microsoft Windows environment. Ability to work independently and achieve high standards of productivity, achieving deadlines and with superior product results. Strong calendaring, organizational, and document management skills required. Proactive approach to solving problems and communicating to leadership. Knowledge of office management systems, procedures, and office equipment required. Excellent time management skills and ability to prioritize work.
