To provide client-centered services that link clients with health care, psychosocial, and other services, ensure timely and coordinated access to medically appropriate levels of health and supportive services and continuity of care including other clinical and non-clinical team members, and coordinate and follow-up with the service/treatment plan.


Duties and Responsibilities


• Conduct initial assessment of medical, psychological, and service needs.
• Develop a comprehensive, individualized service/treatment plan with client input and agreement.
• Coordinate services required to implement the service/treatment plan.
• Monitor clients to assess the efficacy of the service/treatment plan.
• Re-evaluate and adjust the service/treatment plan as needed, but at least every 6 months, if applicable.
• Interact with clients as needed to ensure success of service/treatment plan. Interaction may include face-to-face, phone contact, email or fax.
• Conduct referrals to medical or psychosocial providers as needed.
• Assist clients with obtaining medications that may include assisting with patient assistance programs.
• Assist clients with risk reduction education.
• Educate and review lab standards and results with clients with emphasis on medication adherence.
• Assist client with frequency of medication refills, insurance coverage, and other entitlement programs related to medical and pharmacological assistance.
• Coordinate a medication adherence plan between the client and the pharmacist.
• Provide medication education regarding side effects, adherence, etc., as needed, or in conjunction with the pharmacist.
• Follow-up with client to ensure adherence to medical provider appointments, lab services, and other healthcare/medical providers were kept and are being regularly scheduled.
• Enter case note data into two software programs by following specific requirements of the funder.
• Adhere to the National Monitoring Standards set forth by the Health Resources Services Administration.
• Adhere to the Program Standards of Care for HIV Services set forth by the Marion County Public Health Department Ryan White Services Program.
• Other duties as needed or required by the agency and/or the funder.


Skills and Qualifications


• State of Indiana Professional Licensing Agency-issued Licensed Social Worker (LSW) OR State of Indiana Professional Licensing Agency-issued Licensed Clinical Social Worker (LCSW) and three years' professional experience as a medical care coordinator/case manager
• An RN unencumbered license preferred, ASN or BSN preferred from an accredited college or university
• Experience conducting medical and psychosocial assessments, interventions, follow-ups, and evaluations
• Experience providing clients with risk reduction education
• Experience providing crisis intervention to high need clients with both medical and mental conditions
• Ability to manage duties and responsibilities successfully with limited supervision
• Ability to work independently while also being part of a team
• Ability to travel up to 75% with the Transitional Grant Area (Marion County and all surrounding counties)
• Dependable personal automobile, valid Indiana Driver's License, and on-going automobile insurance coverage are required beginning on first day of employment
• Ability to type at least 40 words per minute
• Ability to formulate sentences that are typed as narrative case notes into two proprietary databases provided by funders
• Proficient in Microsoft Office Suite products (Word, Excel, Outlook)
• Ability to lift up to 25 pounds
• Bilingual Spanish-speaking a plus, but not required
