Basic Function:
In cooperation with HIV positive clients, will provide one on one case management services to a caseload of no fewer than 40 people living with HIV/AIDS.

Full-Time Equivalent/37.5 Hours Weekly

Essential Responsibilities:

Direct Service

• Maintain an active caseload of HIV positive clients
• Interview and assess client's eligibility and need for services
• Formulate care plans and appropriate interventions to address client's concerns
• Assist clients as they move through the service delivery system
• Provide emotional support for clients and their families
• Provide counseling or appropriate referrals to clients in crisis situations
• Act as a liaison between client and all relevant professionals, programs, and resources
• Educate clients and others about HIV disease process and prevention
• Provide 85 hours of billable client interactions per month

Administrative Duties

• Complete necessary paperwork and data entry in a timely fashion including entering case notes within 24 hours
• Collect necessary documentation
• Maintain client records
• Perform effectively within the organization and interpersonally with co-workers
• Attend staff, department, team, and other requested meetings
• Participate in and/or coordinate 1 to 2 additional committees/activities
• Perform other duties as assigned

Program Development

• Develop positive working relationships with other social service, medical, and mental health providers
• Participate in fund-raising and advocacy events as determined by Executive Director and/or Director of Programs


A bachelor's or master's level degree in social work, sociology, psychology, counseling, or nursing is preferred.  Candidates with care coordination or client management experience of two years or more will also be considered as viable applicants.  Candidates will also be considered with some college education if the earned credits are in human behavior, social policy, social welfare, or counseling.

Physical Requirement:

• Ability to sit for extended periods of time
• Ability to relocate in office building several times a day
